Step 1: Obtain an HDHP

To open an HSA, you must have a qualified high deductible health plan (HDHP) in place.
If you don't have a health plan that qualifies for a HSA, the experts at Great Lakes HSA can help!

Please note our application contains fillable fields. Once you open the application in ADOBE Reader, go to the 2nd page. The first FIELD is your "First Name", if you click in the box you can type in your information. Click TAB to continue throughout the form. At any point you can print the application and complete the remaining information by hand.

After you complete the application, you can mail, fax or scan/email it to Great Lakes HSA. It will be processed on the same day! You do not need to return the first page of instructions.
